Missouri College Health Behavior Survey

Hello there!  You have been randomly chosen to test the Missouri College Health Behavior Survey.  We are currently developing a new measure of health assessment directed at college and we need your help to test it.  The questions will ask about your personal attitudes and behaviors towards alcohol, drugs, gambling, safe driving, smoking, etc.  Your testing of the survey is invaluable to our effort to develop an effective instrument to assess other college students like you. 

We ask you to complete two surveys at two separate time points.  For your time, you will be entered into a drawing for one of three $100 gift certificates. 

Consent and eligibility information
You must be 18 years or older to participate in this survey.  Your completion of the survey is voluntary.  This survey will take approximately 10-15 minutes to complete, and your responses are kept anonymous.  They will not be associated with your email address or have any affect on your grades or relationship with the Missouri S&T faculty or staff. You may refuse to answer any questions and stop at any time.  Your complete honesty is appreciated.

 Information about the $100 gift certificates
Once you have finished the survey, you will be entered into a random drawing for one of three $100 gift certificates to the Missouri S&T Bookstore.  This will be a separate page asking for personal information.  There is absolutely no way that your personal information will be connected to the answers on the survey.  If you choose to fill out the survey, please do so by April 18th, 2008, by 5pm. 

There is a risk of experiencing discomfort when disclosing personal information.  You may also become more aware of attitudes and behaviors related to substance use and more aware of programs offered on campus that address substance abuse issues.  If you would like to seek counseling for any reason as a result of this survey, please contact the Counseling Center in Norwood Hall at 573-341-4211.  If you have any questions or concerns that arise during this survey, please contact Ashley Cravens (mattlia@mst.edu), at Student Health Services at 573-341-4225.  If you have any questions regarding human subject research, contact the Missouri University of Science and Technology Campus IRB Office at 573-341-4305.
